Request an Invite

If you are interested in using IQ Health, request an invite by either calling the office that you are seen at or complete the information below. It may take 1-2 business days before you will receive your invitation if you register online.






















How to create and set up an account:


  • Check your inbox for an e-mail from IQHealth.com and open the message.

  • Read over the e-mail and click on the link under step 1.

  • Verify that this is either you or that you are managing this patient's health.

  • Verify your date of birth, enter the security code that you have received from us, and select "Next, Create your Account."

  • You will be prompted to either log in to an existing account or create a new account. If you already have an account, sign in with your e-mail address/username and password on the left side. If not, enter in your e-mail address and select "Sign Up" on the right side.

  • Fill in the form completely.

  • You will now be directed to the home page of your IQ Health account.


Logging in and accessing your account:

  • Direct link to IQ Health log in.
  • From the navigation on the left, you can:
    • ​See our office hours, contact information, and locations
    • Send us general messages or medication refill requests and check your inbox for our responses
    • Request, reschedule, or cancel an appointment
    • View your health profile including clinical record, lab results, and personal information
